CONIFER QUARTER HORSES TRANSPORTED COOLED SEMEN AGREEMENT Date___________ I, _________________________________________________ (mare owner/leasee) have agreed to breed ________________________________________________________________ (name and Reg # of mare) TY in the 2009 breeding season and hereby request that cooled semen be transported for said breeding under the following terms and conditions: 1. First shipment of semen is $ 0.00 (includes overnight standard shipping to mare owner but not return cost of container). All additional shipments will be $ 280.00 per shipment plus shipping (shipping extra Saturdays or to Canada).
2. The stud fee of $1250.00 must be paid in full before the semen will be shipped. 3. A credit card authorization form must be presented to Lloyd Veterinary Medical Center to serve as a refundable semen container deposit. No charges will be made to the card unless the container is not returned or is damaged. 4. The semen container must be returned to Lloyd Veterinary Medical Center the day after the mare is inseminated. A charge of $ 30.00 per day will be charged on the credit card if the container is not received within four days from the day the semen was transported. 5. The breeding must be done under direct supervision of a licensed veterinarian and have a regular and timely teasing program. The mares must be examined by ultrasound to assess breeding status and timely insemination. 6. The mare owner or representative must notify Lloyd Veterinary Medical Center when the mare comes into heat. 7. The mare owner or representative must give Lloyd Veterinary Medical Center a minimum of 48 hours notice when semen is needed. 8. Semen will be collected on Mondays, Wednesdays, and Fridays.
